- Print out the list of required documents and use it as a checklist;.
- Keep a copy of all the documents you send us;
- To complete the documents, we ask for your child's permanent code: this information can be found on the report card;

List of required documents
To apply to Vanguard School, the following documents are required:
Provide the original of the following documents:
- A recent photo;
- Your child's act of birth or the original birth certificate (which we will send back to you by return mail);
- Certificate of eligibility for English language instruction (the original document which we will send back to you by return mail);
- A completed parent questionnaire;
-Elementary: Parent Questionnaire
-High School: Parent Questionnaire
- A completed teachers' questionnaire;
-Elementary: Teacher's Questionnaire
-High School: Teacher's Questionnaire
- A non-refundable cheque of $50 for review of the file.
Provide a copy of the following documents:
- Your child's lastest report card;
- Your child's Individualized Education Plan (IEP);
- A psycho-educational evaluation less than 2 years old, including a WISC-IV or WISC-V with interpretation as well as all other tests that help establish the cognitive profile of your child;
- The most recent speech and language evaluation or intervention report if your child benefitted from such services;
- Various professional reports (remediation, occupational therapy, audiology, neuropsychology, etc.).
